Job summary

Complete Care at Woodlands is seeking a part-time Social Worker (LSW/CSW/LCSW) to join our skilled nursing and rehab team in New Jersey. The Social Worker will manage admissions, discharge planning, family support, and interdisciplinary coordination to support residents through transitions.

The ideal candidate has 1–2 years of healthcare experience, NJ license in LSW/CSW/LCSW, and strong communication and documentation skills. Competitive salary and full benefits are offered.

Qualifications

• Current New Jersey LSW, CSW, or LCSW license required.
• Minimum 1–2 years of healthcare experience, preferably skilled nursing, long-term care, or rehab.
• Experience with discharge planning, Medicaid/Medicare, and MLTS preferred.
• Understanding of CMS regulations and Department of Health survey process preferred.
• Strong communication, documentation, and organizational skills.
• Ability to work independently while collaborating with an interdisciplinary team.

Responsibilities

• Coordinate admissions, discharges, and transition planning.
• Provide emotional support and advocacy for residents and families.
• Participate in interdisciplinary care plan meetings.
• Educate residents and families regarding Advanced Directives.
• Maintain accurate documentation in compliance with state and federal regulations.
• Assist with discharge planning, community resources, and post-acute coordination.
• Support residents through rehabilitation stays and long-term care placement.
