Responsibilities
Coordinate admissions, discharges, and transition planning
Provide emotional support and advocacy for residents and families
Participate in interdisciplinary care plan meetings
Educate residents and families regarding Advanced Directives
Maintain accurate documentation in compliance with state and federal regulations
Assist with discharge planning, community resources, and post-acute coordination
Support residents through rehabilitation stays and long-term care placement
Qualifications
Current New Jersey LSW, CSW, or LCSW license required
Minimum 1–2 years of experience in a healthcare setting, preferably skilled nursing, long-term care, or rehab
Experience with discharge planning, Medicaid/Medicare, and MLTS preferred
Understanding of CMS regulations and Department of Health survey process preferred
Strong communication, documentation, and organizational skills
Ability to work independently while collaborating with an interdisciplinary team
