Job summary

Phoenix Center for Rehabilitation and Pediatrics is seeking a dedicated Social Worker for our geriatric population in a skilled nursing setting. You will plan, organize, develop, and direct the social services program to support residents and families.

The role includes conducting interviews, developing care plans, participating in interdisciplinary teams, and assisting with discharge planning, all within regulatory guidelines. Competitive salary and comprehensive benefits are offered.

Qualifications

• Bachelor’s degree in social work is required.
• Long Term Care / Subacute experience is a plus.
• Demonstrate competence in management and leadership skills.
• Knowledge of state and federal regulations is required.

Responsibilities

• Interview residents and families to identify social and emotional needs.
• Develop and revise written plans of care for each resident.
• Participate in interdisciplinary care plan meetings to address social and emotional needs.
• Participate in discharge planning.
• Refer residents to appropriate social service agencies when needed.
• Provide a positive image of the facility to residents and families.
• Inform residents about rights, personal funds, advance directives, etc.

A skilled nursing facility caring for both the Pediatric and Geriatric populations. We are looking for a devoted and caring Social Worker for our Geriatric population to join our team!

The primary purpose of the position is to assist in the planning, organizing, developing, evaluating, and directing the overall process of the Geriatric social services department.

Responsibilities:

• Maintain and/or improve resident’s ability to control everyday physical, mental, and psychosocial needs
• Interview resident/families to identify the resident’s individual social and emotional needs
• Develop and revise written plans of care (preliminary and comprehensive) for each resident that identifies the needs of the resident and the goals to be accomplished for each need
• Participate in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• Participate in discharge planning
• Refer resident to appropriate social service agencies when the facility does not provide for the services or needs of the resident
• Provide a positive image of the facility to all residents and family members
• Provide residents with information concerning resident rights, personal funds, advance directives, etc.

Requirements:

• Bachelor’s degree in social work
• Long Term Care/ Subacute Experience a plus

• Knowledge of state and federal regulations
