Conduct initial and ongoing assessments of each resident's social service needs
Develop and implement individualized plans of care in collaboration with the interdisciplinary team
Coordinate connections with community resources, including home healthcare agencies, hospices, and other social services
Provide support and counseling to residents and their families, addressing concerns and needs related to social services
Maintain accurate and up-to-date records of resident social service needs and interventions
Participate in staff meetings and contribute to the development of policies and procedures related to social services
Bachelor's degree in Social Work or related field
Licensure as a Social Worker in the State of Pennsylvania
2 years of experience in a healthcare setting
