Job summary

Heart to Heart Hospice seeks a Social Worker to assess psychosocial, grief, financial, and community needs for hospice patients and families. You will provide counseling, guide advance directives, and connect families with resources.

The role requires state licensure and at least one year in clinical care; hospice experience is preferred. This on-site position includes the opportunity to support communities with compassionate care.

Qualifications

• Bachelor's or Master's in social work from a State accredited program.
• Current State license required in accordance with State regulations.
• One year of experience as a Social Worker in a clinical care setting required, hospice experience preferred

Responsibilities

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals.
• Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.
• Facilitate community resources based on patient/family needs.
• Accept clinical assignments that are consistent with education and competence to meet the needs of the patients
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
