Job summary

Metropolitan Jewish Health System, Inc. seeks a hospice social worker to deliver psychosocial support for patients and families across inpatient, home care, and nursing home settings in the Greater New York area.

You will provide pre- and post-bereavement services, coordinate with the hospice IDG, develop and monitor care plans, document assessments, and participate in discharge planning. NY MSW licensure is required; healthcare experience and teamwork skills are essential.

Qualifications

• Graduate from the accredited School of Social Work.
• Minimum of one year experience in Social Work in a healthcare setting.
• Knowledge of Word and Excel preferred.
• New York State MSW licensure.
• Valid NYS Drivers license preferred.
• Ability to work within an interdisciplinary team environment.
• Possess excellent organizational, communication and listening skills.
• Ability to input clinical data proficiently.

Responsibilities

• Provide psychosocial support and Social Work services to Hospice patients and their family members who are at an inpatient unit, nursing home or home care setting.
• Provide pre and post bereavement services.
• Serve as a social work member of hospice IDG and as liaison between hospice, inpatient and nursing home staff.
• Develop the social work plan of care, set goals and make recommendations based on the patient and family needs.
• Maintain accurate and current documentation reflecting palliative social work assessments and interventions.
• Take part in discharge planning when appropriate for patients admitted into inpatient facility.
