Job summary

MJHS in the Greater New York area seeks a Social Worker to deliver psychosocial support to hospice patients and families across inpatient, home care, and nursing home settings. The role involves developing care plans, coordinating with the interdisciplinary team, and guiding families through bereavement phases.

Candidates should have an accredited MSW, at least one year in healthcare social work, and be comfortable working across multiple settings.

Qualifications

• Graduate from the accredited School of social work.
• Minimum of one year experience in Social Work in a healthcare setting.
• Knowledge of Word and Excel preferred.
• New York State MSW licensure.
• Valid NYS drivers license and car required.
• Ability to work within an interdisciplinary team environment.
• Ability to input clinical data proficiently.

Responsibilities

• Provide psychosocial support and other Social Work services to Hospice patients and their families in inpatient, home care/nursing home settings.
• Serve as social work member of hospice IDG and as liaison between hospice staff and inpatient and nursing home staff.
• Develop social work plan of care, goals and recommendations based on patient/family needs for assigned patients admitted to inpatient units or nursing home/hospice residents.
• Develop social work plan of care and discharge plans when appropriate for patients admitted into inpatient facility.

Fee for Service rates:

LMSW rate: $43 per hour

LCSW rate: $46 per hour

In-Person Patient Evaluation: $60 per visit (Telehealth: $40/visit)

Pediatric Specialty: $69 per visit (Telehealth: $40/visit)

Orientation (classroom) $165.00 per day
