Job summary

MJHS is seeking a Social Worker for hospice services, offering LMSW/LCSW rate-based compensation and diverse patient settings. The role covers inpatient, home care, and bereavement support within a not-for-profit health system.

Responsibilities include psychosocial assessments, care planning, and coordinating with IDG teams to ensure holistic patient and family support across care settings. Eligible candidates may hold NY MSW licensure and a valid driver’s license.

Qualifications

• Graduate from an accredited School of Social Work.
• Minimum of one year experience in Social Work in a healthcare setting.
• New York State MSW licensure.
• Valid NYS Driver's license and car.

• Provide psychosocial support and other Social Work services to Hospice patients and their families in inpatient and home care settings.
• Serve as social work IDG member and liaison between hospice staff and inpatient/nursing home staff.
• Develop social work plan of care, goals and recommendations based on patient/family needs for assigned patients.
• Develop discharge plans when appropriate for patients admitted to inpatient facility.

Fee for Service rates:

LMSW rate: $43 per hour

LCSW rate: $46 per hour

In-Person Patient Evaluation: $60 per visit (Telehealth: $40/visit)

Pediatric Specialty: $69 per visit (Telehealth: $40/visit)

Orientation (classroom) $165.00 per day

• You will provide psychosocial support and other Social Work services to Hospice patients and their families in the inpatient, home care/nursing home settings, in pre and post bereavement phases of Hospice service
• You will serve as social work member of hospice IDG and as liaison between hospice staff and inpatient and nursing home staff
• You will develop social work plan of care, goals and makes recommendations based on patient/family needs for assigned patients who are direct admission to inpatient units or nursing home/hospice residents.
• You will develop social work plan of care and discharge plans when appropriate for patients admitted into inpatient facility.

• Graduate from the an accredited School of social work.
• Minimum of one year experience in Social Work in a healthcare setting.
• Knowledge of Word and excel preferred.
• New York State MSW licensure.
• Valid NYS Drivers license and car required.
• Ability to work within an interdisciplinary team environment.
• Ability to input clinical data proficiently.
