Job summary

MJHS in Brooklyn, NY seeks a Hospice Social Worker to provide psychosocial support and bereavement services across inpatient, nursing home, and home care settings. You will collaborate with an interdisciplinary team to develop care plans and document outcomes.

Requirements include NYS MSW licensure and at least one year of healthcare social work experience; Word/Excel skills preferred and a NYS driver license may be required.

Qualifications

• Graduate from an accredited School of Social Work.
• Minimum of one year experience in Social Work in a healthcare setting.
• Knowledge of Word and Excel preferred.
• New York State MSW licensure.
• Valid NYS Driver's license preferred.

Responsibilities

• Provide psychosocial support and Social Work services to Hospice patients and families at inpatient units, nursing homes or home care.
• Provide pre and post bereavement services.
• Serve as a social work member of hospice IDG and liaison between hospice, inpatient and nursing home staff.
• Develop the social work plan of care, set goals and make recommendations based on patient and family needs.
• Maintain accurate documentation reflecting palliative social work assessments and interventions.
• Participate in discharge planning when appropriate for patients admitted to inpatient facility.
