Job summary

Complete Care at Shorrock Gardens in New Jersey is seeking a compassionate Social Worker to support residents and families. You will assess psychosocial needs, advocate for residents' rights, and collaborate with an interdisciplinary team to tailor care.

The role emphasizes empathy, collaboration, and a patient-centered approach within a welcoming facility. LSW or CSW certification is required; prior long-term care experience is preferred.

Qualifications

• Must have LSW or CSW certification.
• Experience in long-term care is preferred but not required.
• Ability to assess psychosocial, emotional, and mental health needs of residents.

Responsibilities

• Identify residents' psychosocial, mental, and emotional needs.
• Complete Social History and Psychosocial assessments.
• Collaborate with interdisciplinary team to promote residents’ rights and well-being.
• Address resident abuse as required by law and licensure.
• Build rapport with residents and peers; work independently or in a team.

Social Worker Qualifications & Responsibilities:

• Must have LSW or CSW
• Previous long term care experience is preferred but not required
• Help to identify resident’s psychosocial, mental, and emotional needs
• Complete Social History and Psychosocial assessment of residents
• Work with interdisciplinary team to promote and protect resident’s rights
• Prevent and address resident abuse as mandated by law and professional licensure
• Ability to build and maintain rapport with residents and peers
• Ability to work independently or part of a group
• Computer knowledge
• Other duties as assigned by Director of Social Services and Administrator
