Job summary

Chelsea Jewish is seeking a Social Worker (JGS – Leavitt Family Jewish Home) for a part-time role at 24 hours per week. The position offers an hourly rate of $30.00–$34.00 based on experience, with preference for long-term care background and MA LSW/LCSW/LICSW licensure.

The role requires fluent English communication and a compassionate, resident-centered approach to psychosocial assessment, counseling, and discharge planning within an interdisciplinary team.

Qualifications

• Bachelor or Master of Social Work (BSW or MSW).
• Current MA LSW, LCSW, or LICSW license.
• Prior experience in a long-term care setting preferred.
• Ability to communicate fluently in English – both verbally and written.

• Conduct comprehensive psychosocial assessments and develop individualized care plans to address residents' emotional, social, and discharge planning needs.
• Provide counseling, intervention, and ongoing support to residents and their families while advocating for resident rights and quality of life.
• Collaborate with an interdisciplinary care team to coordinate services, facilitate care conferences, and ensure resident-centered care.
• Connect residents and families with community resources and post-discharge support services to promote successful care transitions.
