Job summary

Senior Community Care of Michigan PACE, a program of VOANS, is seeking a Social Worker in Lansing, MI to join a compassionate team dedicated to the elderly and their families.

The role emphasizes conducting psychosocial assessments for enrollment, coordinating with the Interdisciplinary Team to develop and review care plans, and providing ongoing support and education about aging, dementia, grief, and PACE services.

Qualifications

  • Master's degree in social work is required.
  • Be legally authorized, currently licensed/registered in the state of employment if applicable.
  • Minimum of one year of experience working with frail or elderly population.

Responsibilities

  • Performs in-person initial assessments for enrollment to obtain psychosocial histories and related information.
  • Coordinates with the Interdisciplinary Team to develop and review the plan of care for each participant.
  • Maintains regular attendance at Interdisciplinary Team meetings and communicates participant changes in the care plan.
  • Provides ongoing support, counsel, and education to participants and families on aging, dementia, grief, and PACE services.

Pay Range:$60,000-$70,000 (LLMSW) $70,000-$80,000 (LMSW)
Schedule:Monday-Friday 8:00 AM-4:30 PM

What You Bring (Requirements)
  • Masters’ Degree from an accredited school of social work
  • Be legally authorized, currently licensed, registered or certified if applicable in the state of employment.
  • A minimum of one year’s experience working with frail or elderly population required.
  • Must have a valid driver’s license and have means of transportation.
What You’ll Be Responsible For
  • Performs in person initial assessments for enrollment of potential Senior Community Care participants to obtain a complete psychosocial history, which may include descriptions of cognitive status, social supports, family dynamics mental health and substance dependency and other issues and needs.
  • Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant. Conducts in person re-assessment of enrolled participants every six (6) months and as needed. Functions as a member of the Interdisciplinary Team.
  • Maintains regular attendance at and participates in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
  • Provides ongoing support, counsel, and education to participants and family regarding a variety of issues, including but not limited to: the aging process, dementia, grief and loss, end of life, disease processes, difficult family dynamics and changing roles, PACE model and PACE health services.
