Responsibilities

• Conduct psychosocial assessments and develop individualized care plans.

• Advocate for residents' rights, needs, and access to appropriate services.
• Coordinate resident admissions and move-in processes, including paperwork and social histories.
• Complete MDS assessments, BIMS, PHQ-9 assessments, consents, NOMNCs, appeals, and other required documentation.
• Maintain accurate and timely records in accordance with organizational and regulatory requirements.
• Help residents and families navigate transitions and access community resources.

Position Summary

St. Paul Elder Services is seeking a compassionate and dedicated Social Worker to support residents and their families by providing emotional support, guidance, resources, and advocacy.
