Job summary

Franciscan Sisters of Christian Charity Sponsored Ministries is seeking compassionate Home Health Aides for the St. Paul at Home program in Kaukauna, WI. This casual PRN role provides in-home support to clients in their own homes or assisted living communities, helping maintain independence and comfort.

Flexibility in scheduling and no nights or weekends are offered. Responsibilities include ADLs, medication reminders, light housekeeping, meals, and companionship, with opportunity to grow within

Responsibilities

• Visit clients in their homes or assisted living facilities on a regular schedule.
• Assist with personal hygiene, bathing, and activities of daily living.
• Support ambulation, transfers, positioning, and ROM activities.
• Provide reminders for self-administration of medications.
• Perform light housekeeping and meal prep.
• Offer companionship and engage in activities with clients.
• Maintain accurate records of services provided and client progress.
• Report changes in client condition to the coordinator.

St. Paul Elder Services is looking for compassionate and dependableHome Health Aides to join our St. Paul at Home team. In this role, you'll provide personalized in-home support to clients living in their own homes or assisted living communities, helping them maintain their independence, comfort, and quality of life.

What You'll Do

Make regularly scheduled visits to clients in their homes or assisted living facilities.

Assist clients with personal hygiene, bathing, and activities of daily living (ADLs).

Help with ambulation, transferring, positioning, and range-of-motion activities.

Provide verbal reminders for self-administration of medications.

Perform light housekeeping, including cooking, cleaning, laundry, changing linens, washing dishes, and taking out trash.

Prepare light meals and snacks.

Provide companionship through conversation, reading, and favorite activities.

Assist with pet care and other needs identified in the client's care plan.

Provide bathing services to Club Gabriel members and community clients.

Maintain accurate records of services provided and client progress.

Report changes in client condition to the Community Services Coordinator.

Build positive relationships with clients, families, and team members.

Strong teamwork, communication, and multitasking skills.

An outgoing personality and enthusiasm for encouraging socialization and participation.

Experience working with seniors in assisted living, memory care, long-term care, skilled nursing, or rehabilitation is a plus.
