Job summary

St. Paul Elder Services in Kaukauna, WI is seeking a dedicated Social Worker to support residents and families with emotional support, resources, and advocacy, ensuring person-centered care and smooth transitions into our community.

This role requires collaboration with a multidisciplinary team in a Five-Star rated facility, offering meaningful impact and opportunities to assist older adults and their families.

Qualifications

• Bachelor's degree in Social Work or related field required.
• Memory care experience strongly preferred.
• Strong communication, organizational, and interpersonal skills.
• Compassionate, empathetic, and committed to resident-centered care.
• Ability to collaborate with the interdisciplinary care team.

Responsibilities

• Conduct psychosocial assessments and develop individualized care plans.
• Provide emotional support and guidance to residents and families.
• Lead care conferences and collaborate with interdisciplinary care team.
• Advocate for residents' rights, needs, and access to appropriate services.
• Coordinate resident admissions and move-in processes, including paperwork and social histories.
• Complete MDS assessments, BIMS, PHQ-9 assessments, consents, NOMNCs, appeals, and other required documentation.
• Maintain accurate and timely records in accordance with organizational and regulatory requirements.
• Help residents and families navigate transitions and access community resources.
