Job summary

Avalon Rehabilitation and Healthcare Center in New Jersey is seeking a licensed Social Worker to provide psychosocial support and care coordination for residents in a SNF/LTC setting. The role focuses on assessing needs, developing plans, and coordinating services to promote well-being and positive outcomes.

The ideal candidate will have a BS in social work (MSW preferred), healthcare experience, and strong communication skills to collaborate with residents, families, and care teams.

Qualifications

• Bachelor's degree in social work or related field.
• Master's degree preferred.
• Experience as a Social Worker in healthcare setting.
• Experience in SNF/LTC settings preferred.
• Strong knowledge of nursing facility regulations.

Responsibilities

• Evaluate residents to assess psychosocial needs, strengths, and limitations.
• Address resident concerns and goals while communicating with families and case managers.
• Build and maintain relationships with residents and families.
• Coordinate care with interdisciplinary teams including therapy and clinical staff.
• Develop and update individualized psychosocial care plans.
• Participate in care planning meetings and ensure continuity of care.
• Provide counseling, emotional support, and resource referrals.
• Ensure compliance with facility policies and documentation standards.

Join our team at Avalon Rehabilitation and Healthcare Center as a licensed Social Worker.

$25/hr.- $40/hr. (All-inclusive based on experience)

Responsibilities for Social Worker

• Evaluate current, new, and prospective residents to assess psychosocial needs, strengths, and limitations.
• Address resident concerns, goals, and emotional needs while maintaining ongoing communication with residents, families, and case managers.
• Build and maintain positive, supportive relationships with residents and their families.
• Coordinate care and collaborate with interdisciplinary teams, including physical therapy and other clinical departments.
• Develop, implement, and update individualized psychosocial care plans for residents.
• Participate in care planning meetings and support continuity of care throughout the resident’s stay.
• Provide counseling, emotional support, and resource referrals as needed.
• Ensure compliance with all applicable facility policies, regulations, and documentation standards.

Qualifications for Social Worker

• Must be able to work full-time hours and maintain a flexible schedule.
• Bachelor's degree required; Master's degree strongly preferred.
• Experience as a Social Worker in a healthcare-related setting required; experience in a nursing home or rehabilitation center strongly preferred.
• Strong knowledge of regulations, policies, and guidelines governing nursing care facilities.
• Excellent communication and interpersonal skills with the ability to interact tactfully with residents, families, staff, and external agencies.
• Ability to work effectively in a fast-paced, team-oriented healthcare environment.
