Highland Park Rehabilitation and Healthcare Center is seeking a dedicated Social Worker to join our full-time team in Chelsea, MA. You will assess residents’ psychosocial needs, develop care plans, and coordinate services to enhance quality of life.

Ideal candidates hold a Bachelor's degree (Master's preferred) with 1+ years in healthcare social work, strong communication skills, and the ability to work collaboratively in a fast-paced environment with residents, families, and staff.

Qualifications

• Bachelor's degree required; master's preferred
• 1+ years experience as Social Worker in healthcare settings required
• Strong knowledge of nursing facility regulations and guidelines
• Excellent communication and interpersonal skills in interactions with residents and families
• Ability to work in a fast-paced, team-oriented healthcare environment

Responsibilities

• Evaluate residents to assess psychosocial needs and develop care plans
• Address resident concerns and communicate with residents, families, and case managers
• Build and maintain supportive relationships with residents and families
• Coordinate care with interdisciplinary teams including physical therapy
• Develop and update individualized psychosocial care plans
• Participate in care planning meetings and ensure continuity of care
• Provide counseling and resource referrals as needed
• Ensure compliance with facility policies and documentation standards

Guided by our core values of Passion, Respect, and Excellence, the Social Worker supports residents and families by assessing needs, developing care plans, and coordinating services that promote well-being and successful care outcomes.

Responsibilities for Social Worker

• Evaluate current, new, and prospective residents to assess psychosocial needs, strengths, and limitations.
• Address resident concerns, goals, and emotional needs while maintaining ongoing communication with residents, families, and case managers.
• Build and maintain positive, supportive relationships with residents and their families.
• Coordinate care and collaborate with interdisciplinary teams, including physical therapy and other clinical departments.
• Develop, implement, and update individualized psychosocial care plans for residents.
• Participate in care planning meetings and support continuity of care throughout the resident’s stay.
• Provide counseling, emotional support, and resource referrals as needed.
• Ensure compliance with all applicable facility policies, regulations, and documentation standards.

Qualifications for Social Worker

• Must be able to work full-time hours and maintain a flexible schedule.
• Bachelor's degree required; master's degree strongly preferred.
• Minimum of 1+ years of experience as a Social Worker in a healthcare-related setting required; experience in a nursing home or rehabilitation center strongly preferred.
• Strong knowledge of regulations, policies, and guidelines governing nursing care facilities.
• Excellent communication and interpersonal skills with the ability to interact tactfully with residents, families, staff, and external agencies.
• Ability to work effectively in a fast-paced, team-oriented healthcare environment.
