Job summary

Hampton Management in New Jersey is seeking a Social Worker to serve as Director of Social Services, supporting residents' emotional and psychological well-being within our community. The role requires a Bachelor's degree in Social Work, at least one year of nursing facility experience, and NJ licensure.

Qualifications

• Bachelor's degree in Social Work or related field.
• Minimum 1 year of experience in a nursing facility.
• License or certification in Social Work in NJ.

Responsibilities

• Conduct social work assessments to identify and address the physical, emotional, and psychological needs of residents.
• Develop and implement care plans to address the identified needs of residents.
• Collaborate with the interdisciplinary team to provide comprehensive care and services to residents.
• Provide counseling and support to residents and their families to promote emotional well-being and independence.
• Maintain accurate and timely records of resident assessments, care plans, and progress notes.
• Participate in care conferences and other meetings to discuss resident care and services.
