Job summary

Lineage Healthcare Group in Berkeley Township, New Jersey seeks a compassionate Social Worker to join our 235-bed facility. You will assess psychosocial needs, develop care plans, and support patients and families.

Collaborating with an interdisciplinary team, you will facilitate communication among patients, families, and providers, address social determinants of health, and contribute to individualized treatment plans.

Qualifications

• Bachelor's or Master's degree in Social Work from an accredited program.
• Minimum of 2 years of experience in a healthcare setting, preferably in social work.
• Strong communication, assessment, and intervention skills.
• Ability to work effectively in a fast-paced environment and prioritize tasks efficiently.
• Strong knowledge of social work principles, theories, and practices.

We are seeking a dedicated and compassionate Social Worker to join our team, a 235-bed facility located in Berkeley Township, New Jersey. As a Social Worker at our facility, you will play a vital role in providing long-term skilled nursing care and short-term post-acute rehabilitation to our patients.

About the Role:

As a Social Worker, you will work closely with our interdisciplinary team to provide high-quality care and support to our patients. Your responsibilities will include:

• Conducting assessments and developing care plans to meet the psychosocial needs of our patients

• Providing counseling and support to patients and their families to address emotional, social, and psychological issues

• Facilitating communication and coordination between patients, families, and healthcare providers

• Identifying and addressing social determinants of health that impact patient outcomes

• Collaborating with other healthcare professionals to develop and implement individualized treatment plans
