HSHS Medical Group in Green Bay, WI is seeking a licensed social worker to join Care Management. This part-time position offers 32 hours per week, 8:00am–4:30pm, with every fifth weekend, supporting behavioral health and social work services across St. Vincent's Hospital and affiliated sites.
Responsibilities include discharge planning and patient-centered care coordination. Required is a Master's in Social Work; or a BSW with five years' clinical social work experience.
Pay Range: $29.55 - $44.32. A successful candidate's actual pay rate will be based on several factors including relevant experience, skills, training, certifications and education.
BLS is required.
In roles which travel is required, a valid driver's license, automobile insurance and own transportation is required.
