• Licensed as a CSW (WI) or LSW (IL) is required; WI CAPSW/CISW/LCSW preferred; IL LCSW preferred.

Social Worker will coordinate and provide social services for patients of all ages in the patient's care setting. Illinois: Licensed as a Licensed Clinical Social Worker (LCSW) is preferred.Eligible temporary Social Worker license while completing the examination requirements for full licensure is required.
