Job summary

Community Healthcare of Texas is actively seeking a Social Worker to provide therapeutic and emotional support to patients and families dealing with serious illness. You will educate families, advocate for patient needs, and collaborate with the care team to align the plan of care with goals and preferences.

The role requires strong counseling and communication skills, a background in social work, and reliability in transportation.

Qualifications

• Master's degree in Social Work required.
• Reliable transportation and a valid driver's license.
• Hospice or healthcare experience preferred.
• Proficient skills using a computer are required

Responsibilities

• Assess the psychosocial needs of the patient and family members

• Contribute to and execute the Plan of Care with the family/patient
• Record daily documentation of visits and contact in accordance with Policy

Community Healthcare of Texas is currently recruiting for a Social Worker.

POSITION SUMMARY

To provide therapeutic/emotional support, education, and resource information to patients and their families; Advocate for patients’ needs/wishes with team members, family members, caregivers, and other providers.

ESSENTIAL FUNCTIONS

• Assess the psychosocial needs of the patient and family members
• Provide therapeutic counseling and emotional support to patients and family members, and education on end-of-life issues
