As a Social Worker at our facility, you will play a vital role in providing long-term skilled nursing care and short-term post-acute rehabilitation to our patients.

About the Role:

As a Social Worker, you will work closely with our interdisciplinary team to provide high-quality care and support to our patients. Your responsibilities will include:
