Qualifications

• Bachelor's Degree in Social Work required.
• Experience in skilled nursing or long-term care preferred.
• Knowledge of discharge planning and resident rights.
• Strong communication, organizational, and problem-solving skills.
• Ability to work with residents, families, physicians and interdisciplinary team.
• EMR experience preferred.

Responsibilities

• Provide psychosocial support to residents and families.
• Conduct social service assessments and document findings.
• Assist with care planning and participate in interdisciplinary team meetings.
• Coordinate discharge planning and transitions of care.
• Facilitate resident and family meetings as needed.
• Assist residents in accessing community resources and support services.
• Address concerns related to resident rights, adjustment, and quality of life.
• Support admissions and orientation processes when necessary.
• Ensure compliance with federal, state, and facility regulations.
