Job summary

Crown Hospice is seeking a Part-Time Hospice Social Worker (BSW/MSW) to join our interdisciplinary team in Missouri. You’ll provide compassionate psychosocial support to patients and families navigating end-of-life care and help develop individualized care plans.

Qualifications

  • Bachelor's or Master's degree in Social Work (BSW or MSW).
  • Current Missouri Social Work license (or eligibility for licensure).
  • Minimum one year of healthcare experience (hospice or home health preferred).
  • Excellent communication, organization, and relationship-building skills.
  • Current CPR certification (or willingness to obtain).
  • Valid driver's license, reliable transportation, and automobile insurance.

Responsibilities

  • Provide emotional support and counseling to patients and families navigating end-of-life care.
  • Complete psychosocial assessments and develop individualized plans to meet each patient's unique needs.
  • Connect patients and families with community resources, advance care planning, financial assistance, and supportive services.
  • Serve as an advocate, educator, and trusted guide throughout the hospice journey.

What You'll Do
