Preferred Hospice of Missouri is seeking a Hospice Social Worker to join our Missouri team. The role involves visiting patients in private homes or facilities, conducting psychosocial assessments, and coordinating with community resources.

The ideal candidate has a Master’s in Social Work (CSWE-approved) or a Bachelor’s in Social Work, with flexibility for on-call crisis needs. EMR documentation and patient education are key responsibilities.

Qualifications

• Strong computer skills for charting in EMR systems and standard office tools.

Responsibilities

• Visit patients where they reside: private home, SNF, ALF, or hospital.
• Complete psychosocial assessment and document in EMR.
• Utilize community resources to assist patients and families.
• Help patients and families cope with death and grief; provide support.
• Explain hospice services and complete admission paperwork; educate community.

Preferred Hospice is a well-established hospice company with 5 offices across the state of Missouri---Dexter, Farmington, St Louis, Columbia, Ozark.

JOB DUTIES

• The Hospice Social Worker visits wherever the patient resides---private home, skilled nursing facility, assisted living facility, hospital.
• The Hospice Social Worker completes a psychosocial assessment on each assigned patient in a timely manner, and documents in the electronic medical record.
• The Hospice Social Worker utilizes community resources for patients, such as in-home services, Meals on Wheels, etc.
• The Hospice Social Worker assists the patient, family and caregiver in coping with issues related to death and dying and participates in grief support as needed.
• The Hospice Social Worker provides information, completes hospice admission paperwork and educates patients, families and the community on all aspects of hospice services.

QUALIFICATIONS

• The successful candidate will have graduated from a School of Social Work approved by the Council of Social Work Education with a Master's Degree (preferred). Bachelor's Degree in Social Work candidates will be considered. (***Other degrees/areas of study do not meet state guidelines and will not be considered.***)
• Work hours are primarily Monday through Friday, 8am to 5pm, with the understanding a crisis can occur at any time. Flexibility with scheduling and hours a must.
• Hourly rate, mileage reimbursement provided. Compensation is based on years of experience.
• Computer skills for charting/EMR, basic use of Word, Excel, Email; smart phone for mileage apps, etc.
• Willing to train the right person. A loving compassionate heart is a must.
