Job summary

Crown Hospice in Victoria, TX is seeking a licensed Hospice Social Worker (LBSW, LMSW, or LCSW) to join our compassionate interdisciplinary team. You will provide emotional support, complete psychosocial assessments, and help families navigate end-of-life care with dignity and comfort.

This full-time role requires a Texas MSW license, at least one year of healthcare experience, strong communication and organizational skills, CPR certification, and reliable transportation.

Qualifications

• MSW from an accredited program.
• Current Texas Social Work license (MSW).
• Minimum of one year of healthcare experience (hospice or home health preferred).
• Strong communication, organization, and relationship-building skills.
• Current CPR certification (or willingness to obtain).
• Valid driver's license, reliable transportation, and automobile insurance.

Responsibilities

• Provide emotional support and counseling to patients and families navigating end-of-life care.
• Complete psychosocial assessments and develop individualized plans to meet each patient's needs.
• Connect patients and families with community resources, advance care planning, financial resources, and supportive services.
• Serve as an advocate, educator, and trusted guide throughout the hospice journey.
• Collaborate with nurses, physicians, chaplains, aides, and other team members to provide holistic care.

What You'll Do
