Community Health Center of Buffalo Inc. in Buffalo, NY is seeking an experienced Social Worker to join our team.

The role focuses on counseling services and case management to address patients' bio-psychosocial needs in a Federally Qualified Health Center setting. The ideal candidate holds a Master’s in Social Work, preferably an LCSW, with strong communication skills and at least two years of related experience.

• Master's Degree in Social Work or related field is required.
• Licensed Certified Social Worker (LCSW) certification is preferred.
• Minimum of two (2) years of experience as a Social Worker; five (5) years preferred.
• Proficiency in MS Word, Excel, and Outlook is necessary.

• Assist patients in completing forms for entitlements, supplemental insurance, and other benefits.

Job Summary

The Social Worker will be responsible for offering counseling services and case management to address the bio-psychosocial needs of patients.

Physical Demands

This role may involve periods of sitting, standing, walking, bending, and stooping. Weekend and evening work may be required.

Salary Range

$63,689 - $93,329 per year depending on license type (LMSW vs. LCSW) and applicable experience.
