SCHEDULE: Monday- Friday: 8:30AM-4:30PM
SUMMARY: Family Service League is seeking a full-time Social Worker for the Assertive Community Treatment (ACT) program. The Social Worker delivers services to individuals who are diagnosed with severe mental illness and whose needs have not been met by more traditional service delivery approaches. The Social Worker will be given an agency vehicle to use for home visits with the clients. Travel is required.

QUALIFICATIONS:
Master’s Degree required.
LMSW, LCSW required
1 year of experience working with the severe and persistent mental illness (SPMI) population required.
Ability to work independently and as part of a multi-disciplinary team required.
Excellent interpersonal and communication skills, both verbal and written required.
Proficient computer skills, including Microsoft Office and Electronic Health Records/Documentation, required.
Ability to multitask and prioritize in a fast-paced environment.
Valid and clean New York State Driver’s License required.
PHYSICAL REQUIREMENTS: This position requires sitting for long periods of time and travelling to conduct home visits.
