Job summary

Family Service League Inc. in Riverhead, NY is seeking a full-time, Master’s-level Social Worker for our School Based Program.

The role provides individual, family, and group therapy for adolescents with a wide range of DSM diagnoses within an educational setting connected to an outpatient mental health clinic. The candidate must hold LMSW, LMHC, LCSW or a related licensed clinical degree and have at least one year of behavioral health experience; school-based counseling is strongly preferred.

Qualifications

• Master’s degree in relevant human service field required.
• LMSW, LMHC, LCSW or related licensed clinical degree or limited permit required.
• At least one year of experience in a behavioral health setting required; school-based counseling preferred.
• Proficient computer skills including Microsoft Office and Electronic Health Records.
• Excellent interpersonal, verbal and written communication skills required.
• Ability to multitask, problem solve, make clear decisions, and work independently or as part of a team.
• Bilingual in Spanish preferred, but not required.

Responsibilities

• Provide individual and group therapy to adolescents in a school-based setting, in-person and via telehealth.
• Complete all clinical documentation in electronic medical records, including progress notes and assessments.
• Coordinate with school personnel, parents/guardians, and community providers.
• Maintain liaison with referral sources and discharge planning.
• Attend routine supervision and team meetings, and support PQI/COA accreditation.
• Provide services in outpatient clinic when school is not in session or as needed.

One evening shift/ availability required.

SUMMARY:

Family Service League is seeking a full-time, Master’s-level Social Worker for our School Based Program in Riverhead, NY. RESPONSIBILITIES:

• The Social Worker will provide individual and group therapy to children/ adolescents with full range of DSM diagnoses in a school-based setting. Direct service will be provided both in person and via telehealth
• Complete all clinical documentation in accordance with NYS Office of Mental Health in an electronic medical record, including progress notes, comprehensive assessments and Individual Actions Plans.
• Maintain ongoing communication and collaboration with school personnel, parents/guardians, and community providers.
• Provide collateral contacts with all relevant referral sources, including family members.
• The Counselor will coordinate linkage to community resources and discharge planning.
• Attend routine supervision/team meetings.
• The Counselor will understand and demonstrate a commitment to Performance Quality Insurance (PQI) and accreditation with the Council on Accreditation for Children and Family Services (COA).
• Provide additional support/services to the clinic as needed.
• When school is not in session, or as needed during the year, provide services in the outpatient clinic.
• All other duties as assigned.

Master’s degree in relevant human service field required.

At least one year of experience working in a behavioral health setting required. Experience providing school-based mental health counseling is strongly preferred.

Proficient computer skills, including Microsoft Office and Electronic Health Records required.
