Family Service League Inc. Bay Shore, New York, United States seeks a full-time Bilingual, Spanish-Speaking Case Manager for the Children’s Health Homes program. The Case Manager coordinates care for youth with mental illness or chronic conditions and develops strength-based plans with clients through regular meetings.

SUMMARY:
Family Service League is seeking a full-time Bilingual, Spanish-Speaking, Case Manager for the Children’s Health Homes program. The Case Manager will offer care coordination to children and young adults with mental illness and/or chronic medical conditions. The Case Manager will be responsible for conducting meetings with clients to develop strength assessments and individualized planning, as well as support them with their goal achievements. Travel is required.

Evening hours required for at least 2 days a week

QUALIFICATIONS:
Bachelor’s Degree in Social Work with 2 years of related experience or Master’s Degree with 1 year of related experience required.
Excellent interpersonal and organizational skills required.
Strong verbal and written communication skills required.
Ability to solve problems and work as part of a team required.
Valid and clean NYS Driver’s License required.
PHYSICAL REQUIREMENTS:
This position requires sitting for long periods of time and travelling to conduct home visits.
