Job summary

Family Service League Inc. in Bay Shore, NY seeks a full-time Social Worker for the Assertive Community Treatment (ACT) program. You will deliver services to individuals with severe mental illness and meet goals through individualized plans, with travel using an agency vehicle.

The role requires LMSW/LCSW and one year of relevant experience, plus strong communication and documentation skills. A comprehensive benefits package and sign-on bonus apply.

Qualifications

• Master’s Degree required.
• LMSW or LCSW license required.
• Minimum 1 year of experience with severe mental illness (SPMI).
• Ability to work independently and in a multidisciplinary team.
• Excellent verbal and written communication skills.
• Proficient with Microsoft Office and EHR documentation.

Responsibilities

• Educate clients and families about managing severe mental illness and goals.
• Conduct daily home visits using the agency vehicle.
• Create individual treatment plans based on client needs.
• Complete client assessments, relapse plans, and housing applications.
• Develop goals toward medication, housing, and employment plans.
• Provide evidence-based treatment to clients with SPMI.
• Connect clients to clinics and community mental health resources.
• Perform crisis intervention using crisis phone after hours/weekends as needed.
• Conduct group meetings with clients and families to rebuild trust.
• Travel using the agency vehicle; perform other duties as assigned.
