Job summary

Mentor Maryland, a part of the Sevita family, is seeking a Social Worker for Children in Treatment Foster Care to join our mission-driven team. The role focuses on coordinating and delivering case management and counseling services to youth.

You will manage caseloads of treatment foster care children from 0 to 21, develop and coordinate treatment plans, and perform weekly home visits. A Bachelor's or Master's in Social Work is required; valid driver's license and ability to work in a team are

• Bachelor’s or Master’s in Social Work required by the state.
• Self-motivated and detail-oriented with ability to multi-task.
• Valid driver’s license with access to a registered vehicle and proof of insurance.

Responsibilities

• Provide case management to an assigned caseload of treatment foster care children, ages 0 to 21.
• Lead development and coordination of child's treatment and service plan.
• Make weekly home visits to each child on caseload.
• Provide monitoring and consultation to assigned foster parents (Mentors).
• Maintain records in accordance with corporate, contractual, and regulatory standards.
• Assist with legal, licensing, regulatory and certification activities as needed.
• Assist with emergency coverage and crisis intervention support, including on-call rotation.

Social Worker for Children in Treatment Foster Care

Do you have an interest in working with children/teens with mental health needs and experiences of trauma? Bring your skills to a team-based workplace that puts people first. As a full-time Clinical Coordinator for treatment foster care youth, you will advance the company’s mission of providing quality service by facilitating and providing case management and counseling services.

• Provide case management to an assigned caseload of treatment foster care children, ages 0 to 21.
