Job summary

Georgia Mentor, a part of the Sevita family, provides Residential and Treatment Foster Care services to youth facing out-of-home placements and transitions. We are seeking a Case Manager to coordinate services, monitor care quality, and support families within our mission-driven team.

Full-time role with competitive pay: $42,000–$44,000 depending onBachelor’s/Master’s level, plus a comprehensive benefits package. Join us to make a meaningful impact in youth lives.

Qualifications

• Bachelor’s degree in human services or related field.
• One year of work-related experience with individuals with IDD, brain injury, or at-risk youth preferred.
• Equivalent combination of education and experience.
• Current driver's license, car registration, and auto insurance.
• Licensure and training as required by state (CPR, First Aid, Behavioral Intervention Techniques, etc.).
• Compassionate approach and a commitment to quality.

Responsibilities

• Coordinate the development, review, and implementation of services, case management, behavior intervention plans, and treatments provided to individuals served.
• Oversee all services provided to individuals and their families, monitor quality of care delivered, ensure compliance with contractual terms, and monitor regulatory program compliance.
• Assist in the development of quality services and engaging activities that meet the individual served needs.
• Build and maintain relationships with families and external case managers.
