Job summary

Sevita's Mentor Maryland, a part of the Sevita family, is hiring a Social Worker for Children in Treatment Foster Care. This full-time Clinical Coordinator will provide case management and counseling services to youth in treatment foster care.

You'll lead development and coordination of treatment plans, make weekly home visits, support foster parents, maintain records for regulatory standards, and assist with licensing tasks. A BS/MSW is required and a valid driver's license.

Qualifications

• Bachelor’s or Master’s in Social Work required by the state.

Responsibilities

• Provide case management to an assigned caseload of treatment foster care children, ages 0 to 21.
• Lead development and coordination of child’s treatment and service plan.
• Make weekly home visits to each child on caseload.
• Provide monitoring and consultation to assigned foster parents (Mentors).
• Maintain records in accordance with corporate, contractual, and regulatory standards.
• Assist with legal, licensing, regulatory and certification activities as needed.
• Assist with emergency coverage and crisis intervention support as needed, including participation in rotational on-call system
