Job Summary and Qualifications

The Social Worker (SW) promotes patient-centered care by evaluating psychosocial needs and coordinating community services for patients. As part of a multidisciplinary team, the SW acts as a liaison with community resources and post-acute care providers, facilitates discharge planning, and supports patient movement across care settings. The role primarily involves Med/Surg units but requires flexibility to work in all hospital areas.

Responsibilities include:

• Collaborating with healthcare team members to identify and address patient needs.
• Identifying barriers to discharge and recovery, including financial, emotional, psychosocial, medical, and spiritual issues.
• Performing assessments and developing discharge plans for high-risk patients.
• Ensuring discharge plans align with patient care needs, reassessing as necessary.
• Participating in care planning and proactive discharge planning.
• Educating patients and caregivers about community resources and post-acute care benefits.
• Facilitating team meetings and assisting patients and families with decision-making, respecting their preferences and values.

Qualifications:

• A Master’s Degree in Social Work is required.
• Licensure as LMSW or LCSW is highly preferred.
• 3-5 years of hospital patient care experience is preferred.
