Qualifications

• Master's: Social Work (Required)
• Three (3) years of experience as a social worker in a clinical setting.
• Active participation in social work professional organizations preferred.
• Certified Clinical Transplant Social Worker (CCTSW) credential preferred, where applicable. LCSW preferred.

Responsibilities

• Leads the handling and resolution of complex issues and complaints.
• Responsible for program's patients' social needs including discharge planning.
• Evaluations to include psychosocial assessments to determine candidacy and continued work with patient and family.
• Provides direction, orientation, training, coaching, and mentoring to staff.
• Assesses quality of services and facilitates staff development programs.
• Ensures staff compliance with policies, procedures, and protocols.
