Job summary

Memorial Healthcare System is seeking a Social Worker to provide discharge planning and care coordination within the acute care setting. You will conduct discharge assessments, support physician consults, and coordinate post-acute services to ensure safe, timely patient transitions.

Working across units, you will promote continuity of care and adherence to workflows while responding to the needs of patients and families.

Qualifications

• Bachelor's in Social Work required.
• Minimum of one year of professional work experience in Social Work.
• Strong communication and assessment skills.

Responsibilities

• Participates in interdisciplinary rounds and case reviews to evaluate case management and progress.
• Consults with healthcare team to promote adherence to treatment plans.
• Interviews and assesses patients, families, and caregivers to determine needs.
• Determines and arranges for internal and external services/interventions.
• Assists with discharge planning and processes.
• Identifies resources and provides referrals as part of the discharge plan.
