Job summary

SUNY Downstate Health Sciences University seeks a full-time TH Social Worker III, Clinical Supervisor for the HEAT Program in Brooklyn, NY. The role supports adolescents and young adults living with HIV or at elevated risk through psychosocial assessment, counseling, and care coordination.

The incumbent collaborates with a multidisciplinary team to develop individualized care plans, assist with discharge planning, and advocate for community resources.

Qualifications

• Master’s Degree in Social Work (CSWE accredited) or equivalent education for licensure.
• Current NYS licensure as LMSW or LCSW.
• Strong interpersonal, communication, organizational and problem-solving skills.
• Ability to collaborate effectively with multidisciplinary teams.
• Experience with discharge planning and care coordination preferred.
• EMR/EHR experience preferred.

Responsibilities

• Conduct psychosocial assessments of patients and families.
• Develop, implement, and evaluate psychosocial intervention plans.
• Provide supportive counseling, crisis intervention, and care coordination.
• Participate in rounds, huddles, conferences and care planning meetings.
• Coordinate with physicians, nurses, case managers and other providers.
• Assist with safe, timely discharge planning and post-acute placements.
• Coordinate with external agencies for referrals and placements.
