Qualifications

• Ability to work across inpatient, outpatient, emergency, pediatric, and other settings.

Responsibilities

• Conduct psychosocial assessments of patients and families to identify psychosocial, financial, and environmental factors affecting health and discharge planning.
• Develop and evaluate individualized psychosocial interventions with the care team.
• Provide crisis intervention, supportive counseling, and education to patients and families.
