The Social Worker coordinates with the interdisciplinary team to plan timely discharges, link patients to community resources, and support transitions of care, while ensuring compliance with licensure and hospital policies.

Qualifications

• Master’s Degree in Social Work (MSW) from CSWE-accredited program.
• Current New York LMSW or Licensed Clinical Social Worker (LCSW) licensure.
• Knowledge of psychosocial assessment, crisis intervention, advocacy, and care coordination.
• Ability to develop discharge plans and coordinate community resources.

Responsibilities

• Conduct psychosocial assessments identifying factors affecting health, recovery, and discharge.
• Develop, implement, and evaluate individualized psychosocial intervention plans with the care team.
• Provide counseling, crisis intervention, and bereavement support within licensure scope.
• Participate in rounds, huddles, and care planning to address psychosocial needs.
• Coordinate with care managers, physicians, and community services to support transitions of care.
• Assist with timely, safe discharge planning and post-acute placements.
• Document assessments and interventions in the EMR; ensure confidentiality.
