Qualifications

• MSW required; 1 year of clinical social work in health care.
• Credentials from multiple states required at hire; DC licensure within 6 months.
• MD and VA licensure within 6 months.

Responsibilities

• Collaborate with health care team to coordinate cost-effective services aligned with the plan of care.
• Complete psychosocial assessments to evaluate goals, support, resources, and status.
• Develop and monitor care plans with nurse case manager to optimize functioning.
• Identify resources and advocate for community supports; ensure timely access.
• Document case management activities per organization standards.
