Advocate Health seeks a full-time Case Manager Social Worker to join the Nursing: Care Coordination team at Wake Forest Baptist Medical Center. The role emphasizes discharge planning, psychosocial assessments, and coordinating referrals to ensure optimal patient outcomes.

Requires a CSWE-accredited MSW and licensure; ACM-SW certification is preferred. The position includes a 40-hour work week, Monday–Friday, with benefits eligibility and a competitive pay range.

Qualifications

• Master's in Social Work from CSWE-accredited program.
• Clinical licensure desirable or provisional status.
• ACM-SW certification preferred.
• Experience with psychosocial assessments and discharge planning.

Responsabilités

• Identify patients who would benefit from case management.
• Assess data via interviews and objective evaluation of patient needs.
• Complete psychosocial assessments to identify emotional, social, and environmental needs.
• Develop and implement psychosocial treatment plans including therapies as needed.
• Initiate internal and external referrals specific to patient needs.
• Coordinate discharge planning with the multi-disciplinary team to drive progression.
• Document in the EMR according to departmental standards and payer practices.
