Responsibilities

• Identify patients who would benefit from Case Management interventions.
• Assess data via interviews and evaluation of patient needs.
• Complete psychosocial assessments to identify emotional, social and environmental needs.
• Develop and implement psychosocial treatment plans, including crisis intervention and family therapy as needed.
• Initiate internal and external referrals.
• Maintain EMR documentation and knowledge of payer practices.
• Coordinate discharge planning with the multidisciplinary team.
