Advocate Health is seeking a Social Work Case Manager to coordinate psychosocial care for patients at Wake Forest Baptist Medical Center in Winston-Salem, NC. The role includes assessments, planning, and discharge coordination within a busy hospital setting.

The position requires an MSW, CSWE accreditation, and licensure considerations; ACM-SW certification is preferred. Excellent interdisciplinary collaboration and knowledge of payer practices are essential.

Qualificações

• Master’s degree in Social Work from CSWE-accredited program.
• Clinical licensure desirable or provisional status.
• ACM-SW certification preferred.
• Healthcare social work experience in a clinical setting desirable.
• Strong assessment, communication, and discharge planning skills.

Responsabilidades

• Identify patients who would benefit from Case Management interventions.
• Assess data via interviews and objective evaluation of patient needs.
• Complete psychosocial assessments per department and professional standards.
• Develop and implement psychosocial treatment plans with appropriate modalities.
• Initiate internal and external referrals for patient needs.
• Maintain knowledge of federal, state, and local regulations.
• Coordinate discharge planning with the multidisciplinary team.
