Job summary

Ascension Saint Joseph in Chicago is seeking a social work professional to coordinate the interdisciplinary plan of care from admission to discharge, incorporating patient and family preferences (LSW required, LCSW preferred).

You will monitor patient care through assessments and medical records, advocate for resources and post-discharge options, and maintain ongoing dialogue with supervisors and other health providers to ensure the health plan is effectively implemented.

Qualifications

• Clinical Counselor or Social Worker licensure in Illinois required prior to hire.
• LSW required; LCSW preferred.
• Social Worker or Counselor credentialing through Illinois Department of Financial and Professional Regulation.

Responsibilities

Education

High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required
