Job summary

Northwestern Medicine seeks a licensed social worker (LSW) to join our inpatient care team in Chicago. You will perform psychosocial assessments, coordinate discharge planning, and support patients and families throughout the care journey.

In collaboration with physicians and other professionals, you will ensure ethical, patient-centered care, provide crisis intervention when needed, and help connect patients to community resources for post-discharge continuity.

Qualifications

• Master's Degree in Social Work from CSWE-accredited program.
• Strong interpersonal and communication skills.
• Ability to prioritize and manage caseloads effectively.

Responsibilities
