Job summary

Swedish Health Services is seeking a full-time Social Work Case Manager to provide a full range of social work services including psychosocial assessment, treatment planning, therapeutic interventions, discharge planning, crisis intervention, and resource referral.

Partner with the patient and their support system, as well as interdisciplinary teams, both internal and external to the organization, to improve patient care through the effective utilization and monitoring of health care resources.

Qualifications

  • Master's degree in Social Work.
  • 1 year of experience as Social Worker or similar.
  • Washington State Social Worker Independent Clinical License.
  • 1 year of experience in acute medical or mental healthcare in an inpatient or outpatient setting.

Responsibilities

  • Provide psychosocial assessment and treatment planning.
  • Deliver therapeutic interventions and discharge planning.
  • Perform crisis intervention and resource referral.
  • Collaborate with patient, family, and interdisciplinary teams.
  • Monitor and document progress toward clinical, financial, and resource outcomes.

Applicants must have:
  • Masters degree in Social Work
  • 1 year experience as Social Worker or similar
  • Washington State Social Worker Independent Clinical License
  • 1 year of experience in the areas of acute medical or mental healthcare in an inpatient or outpatient setting.

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
