Job summary

Providence Swedish First Hill in Seattle is seeking a Nurse Case Manager to coordinate care and services for selected patient populations across the acute care episode. You will work with interdisciplinary teams to optimize patient care, resource use, and outcomes, taking a leadership role in achieving clinical and financial goals.

The role requires a Bachelor's in Nursing, WA RN license at hire, and at least 3 years of acute care nursing experience; pursuing a BSN within three years is required

M-F; One Weekend Rotation a month

The nurse case manager coordinates the care and service of selected patient populations through the acute care episode, across the continuum. Works collaboratively with inter-disciplinary teams, both internal and external to the organization, to improve patient care through effective utilization and monitoring of healthcare resources and assumes a leadership role to achieve desired clinical, financial, and resource outcomes.

Required Qualifications:

• Bachelor's Degree: Graduate of an accredited Bachelors Nursing degree program or completion of an accredited Nursing program with a bachelor's degree or higher in a health care field, or
• Applicants without a BSN degree must actively pursue and attain a BSN within three years of hire. Continued employment by Swedish beyond three years is contingent upon attaining the BSN degree.
• Upon hire: Washington Registered Nurse License
• 3 years Registered nursing experience in an acute care hospital.

Preferred Qualifications:

• 1 year Hospital case management experience.
