Job summary

Providence Swedish seeks a patient-focused care management leader to supervise day-to-day clinical and administrative operations of the care management team in Seattle, WA. This role ensures coordination across nursing, social work, and care coordinators to deliver high-quality care while managing costs.

Required Washington RN license and at least 5 years of clinical nursing experience, including acute care settings.

Qualifications

• Washington Registered Nurse License is required at hire.
• Minimum of 5 years clinical nursing experience including acute care hospital and/or ambulatory clinic.
• Strong supervisory experience and ability to lead care management staff.

Responsibilities

• Oversee day-to-day clinical and administrative operations of the care management team.
• Supervise CM staff including nursing, social work, and care coordinators to meet organizational goals.
• Collaborate with inpatient/ambulatory clinicians, SMG and PCN leaders to support cost-effective, high-quality services.
• Provide leadership to ensure staffing, performance, and quality improvement within care management.

Required Qualifications

• Upon hire: Washington Registered Nurse License
• 5 years Clinical nursing, including acute care hospital and/or ambulatory clinic experience.

Preferred Qualifications

• Bachelor's Degree
• 3 years Case Management experience including care management, care coordination, discharge planning, or population health.
• 2 years Progressive supervisory experience.
