Job summary

Olympic Medical Center in Port Angeles, WA seeks a Social Work Care Manager I to work with an interdisciplinary team to facilitate discharge planning and address patients’ psychosocial needs. You will perform mental health assessments, conduct SBIRT, coordinate with physicians and nursing staff, and support families through complex care transitions.

Requirements include a Master of Social Work, BLS within 30 days, SBIRT certification within six months, and prior medical social work experience

Qualifications

• Masters of Social Work required.
• Previous experience in medical social work preferred.
• SBIRT certification required within six months of hire.
• Basic Life Support (BLS) certification required within 30 days of hire.

Responsibilities

• Facilitate discharge planning and address psychosocial needs for patients at risk of adverse outcomes.
• Perform mental health assessments and crisis interventions as needed.
• Coordinate with nurses, physicians and other staff to determine social work involvement.
• Conduct cognitive function screenings and SBIRT interventions when appropriate.
